Health Form Policies

Patients often have forms that need to be completed for schools, daycares, camps, teams, and other organizations. This is especially common with sports and camp physicals. We make every effort to complete them in a timely fashion, ideally during your child’s office visit or well-child exam. We are glad to complete these forms for you, with the following guidelines:

  1. It is best by far to complete any forms at the time of your child’s visit, usually a well-child visit. Please make every effort to plan accordingly.
  2. You should bring any necessary forms with you to your visit, especially sports clearance forms.
  3. We do not currently charge for form completion after a visit.
  4. Forms brought to an office visit will usually be completed at the visit. Forms brought in after a visit will be completed within 3-5 business days.
  5. Forms will usually be completed within five business days. We try to complete forms as quickly as possible, and they are usually done within a few days.
  6. We cannot complete blank forms. You must complete your portion of the form first and sign where appropriate. This includes the history portion of the sports physical form and the Boy Scout camp form. You must also complete the name, address, and similar portions first.
  7. Most forms may require a physical/well-child visit in the past 12 months. We will strictly comply with what the form says it requires. Forms will be dated based on the date of the last physical and will expire 12 months after that visit, not 12 months after the form is completed.
  8. If you need a form completed a later date, we may have additional questions for you or additional exams may be needed, which may require an additional visit.
  9. Forms are completed for those whose accounts are in good standing. If your account is delinquent, you will have to bring it up to date first.
  10. Due to privacy regulations, forms have to be picked up by the child or parent in our office. They cannot be mailed or faxed to non-medical locations.
  11. Forms are based only on exams or medical information in our office. We will not co-sign forms completed by other examiners, or fill out a new form based on an exam at another office, school, or retail based health clinic.
