Welcome to the CCP Family! We are honored and delighted that you would choose us as your health partner and trust us with the care of your child and family. We take that trust and responsibility very seriously. We want to be a true medical home for your children and your family, and are on a quest to develop the ideal pediatric practice. We believe every child is a gift from God, and try to treat every child as we would our own children.

Newborns

We love babies! If you are currently pregnant, please see our information for expecting parents. We invite you to schedule a free visit to meet with Dr. Pulliam and see our practice. If you are interested in a more thorough visit, with pregnancy, birth, or breastfeeding education and planning, we also offer prenatal consultations. See here for more information.

If you are having your baby at Piedmont Henry and you list Dr. Pulliam as your baby’s doctor, then he will see your baby within 24 hours of delivery. If you are having a baby another hospital and wish to use our practice, an on-call doctor at that hospital will see your baby in the first 24 hours. We want you to follow-up with us within 1-3 days of discharge. Just call us for an appointment at (770) 389-4543. You can fill out your forms in advance, or just complete it when you arrive with your baby for your first appointment. You may want to arrive 10-15 minutes early for your first appointment.

Most insurance plans require that babies are added by 30 days old; most don’t cover babies under the mother’s plan anymore. You need to be sure that your baby has been added to your plan before the baby is 30 days old or you could face significant out-of-pocket expenses.

General Information for New Patients

We are on most insurance plans; a complete list is available here; please call our office if you have any questions. We will need to verify your insurance coverage prior to or at your first visit. If you can get us your insurance information prior to your first visit, then this will shorten your wait time at your visit.

If your plan requires you to identify a Primary Care Provider (PCP), then you need to call your insurance company and switch to us as your PCP prior to your visit. Please be aware that this can take up to 30 days.

If you are transferring to our practice, we need a copy of your child’s medical records, especially if you are scheduling a well-child exam. We need these records before your child’s physical. See medical records section below. You should also bring a copy of your child’s vaccine record to your child’s first visit. You can use our medical records request form to ask your child’s doctor to send us a copy of the records.

You can fill out new patient forms and send them to us or bring them with you to your first visit. Our fax number is (770) 474-0566. If you fax them, please bring the original to your first visit. Also please fax us a copy of your insurance card, front and back sides.

Here is a list of the information we need from you:

  • New patient forms
  • Current insurance card, front and back, for the insurance you are going to add the baby to
  • A copy of your child’s medical records (use this form to request your child’s records, or the form at your previous doctor’s office)

If you are not providing any of this information prior to your visit, please arrive 20-30 minutes early for your first appointment so that we can complete your information and verify your insurance coverage.
